Advanced Course in Rectal cancer

ADVANCED COURSE ON RECTAL CANCER
8-10 SEPTEMBER 2008, THE HAGUE, THE NETHERLANDS

Preceding the 14th ESSO Congress, ESSO and ESO jointly organise a unique educational programme on rectal cancer open to both senior residents in the last two years of training and junior surgeons within five years after boarding.

Course objectives

Based on the knowledge of TME surgery and imaging before the operation it is possible to asnwer the 10 following questions after the advanced course:

  • How does staging influence clinical decision makking?
  • Where do I damage the autonomic nerve plexus?
  • How to diagnose and treat lymph node metastases outside the mesorectal envelope?
  • How and when can I minimize the procedure?
  • How do I position the patient for the perineal phase?
  • How do I perform a wide excision for low rectal cancers?
  • How do I deal with the loss of a pelvic floor after a wide perineal resection and how do I correct a perineal hernia?
  • How do I estimate the need of a defunctioning colostomy and how do I  treat leakage of the anastomosis?
  • How do I treat a patient with rectal cancer and synchronous liver metastases?
  • How do I balance the advantages and disadvantages of adjuvant treatment (numbers needed to treat)?
Ideal candidate
  • Senior resident (last two years of training)
  • Junior surgeon (within five years after boarding)
  • Good in English (written and spoken)
  • Is, or willing to become, an individual ESSO member

Application procedure 

Application must include:

  • Complete Application Form
  • Letter describing motivation for attending
  • Short CV (maximum 2 pages) with list of recent publications
  • Supporting letter from head of department
  • Abstract (1 page) with proposed presentation of a clinical case relating to one of the main themes of the Advanced Course

TUESDAY 9 SEPTEMBER

SURGICAL TREATMENT

Lecture: The algorithm of treatment of a patient with locally advanced rectal cancer

Workshops:
•Positioning the patient
•Technique of a wide perineal resection
•Filling and reconstruction
•Perineal hernia’s
Symposium: Is less possible
•Indications for TEM
•Laparoscopic rectal resection
•Partial mesorectal excision
Lecture: Factors influencing suture dehiscence
Workshops:
•Stapling techniques
•Early detection of leakage
•Treatment of leakage
•Stoma’s: colostomy, ileostomy, timing of
closure

WEDNESDAY 10 SEPTEMBER

NUMBERS NEEDED TO TREAT

•Neoadjuvant radiation therapy Is indicated in (almost) all cases Presents with major morbidity
•Adjuvant chemotherapy Should be offered like in colon cancer Is not proven to be efficacious

Workshops on the treatment algorithm of patients with primary rectal cancer and distant metastases followed by a plenary session in trying to define the optimal strategy.

Lecture: Experiences from a multidisciplinary team.
